PREPARATION

• Review workplace specific safety procedures with students and placement supervisor
• Confirm with the employer that the student will receive training on the required  safety equipment to be worn at placement and the location of first aid station, fire extinguisher, etc.
• Ensure the Work Education Agreement is completed and on file at the school and at the Education Centre
• Inform students placed as teacher assistants that they do not have medical coverage through WSIB
• Recommend all students involved in cooperative education or experiential learning programs purchase Student Accident Insurance

INJURIES REQUIRING FIRST AID ONLY AT THE WORKPLACE

For all students involved in cooperative education or experiential learning programs

For students placed as teacher assistants and require medical treatment by the hospital, dentist, etc

• Inform parent/guardian of the injury
• Complete the OSBIE Incident Report form available from the school office (list only the facts!)
• Return the completed OSBIE Incident Report form to the school office
• Inform the Experiential Learning Coordinator, Curriculum Services at the Education Centre of the injury

INJURIES REQUIRING MEDICAL TREATMENT BY THE HOSPITAL, DENTIST, ETC

Not applicable to students placed as teacher assistants

• Inform parent/guardian of the injury
• Complete and fax the following forms to Curriculum Services at the Education Centre (613-966-6023)                                  within 24 hours of the injury
1. WSIB Form 7 (Employer’s Report of Injury/Disease) 
2. Worker’s Claim Consent Form (G-7)
3. Work Education Agreement
• Complete the OSBIE Incident Report form available from the school office (list only the facts!)
• Return the completed OSBIE Incident Report form to the school office
• If the student must modify duties due to the injury, as directed by a doctor, instruct the student to arranged for a health professional to complete the Functional Abilities Form for Timely Return to Work (G-6).  This form must be completed before the student returns to placement.  Upon completion, fax this form to Curriculum Services at the Education Centre (613-966-6023)

FOLLOW UP FOR ALL WORKPLACE INJURIES

• Review any safety factors or training with the student and placement supervisor in order to minimize the possibility of reoccurrence
